The following forms will need to be completed in order to tryout for the basketball team (links to all forms are on this website):

• 1. Knox County Updated Physical Form (MUST BE FILLED OUT BY A PHYSICIAN AND DATED ON OR AFTER APRIL 15TH, 2024……THIS IS KNOX COUNTY POLICY)
  2. Concussion Information Packet (Filled out by parent or guardian AND student)
  3. Sudden Cardiac Arrest Form (Filled out by parent or guardian AND student).
• In addition to completing and turning in the required forms by the due date, a student must have a minimum 2.0 GPA and BEHAVIOR CLEARANCE FROM ADMINISTRATION in order to be an eligible candidate for the basketball team.
  ALL FORMS ARE DUE BACK TO COACH ZORIO BY FRIDAY SEPTEMBER 27TH. Please do not wait until the last minute to turn in forms (as forms need to be verified and processed before tryouts begin). Please check the website for updates regarding specific tryout dates and times.

The first days of tryouts will be by grade level this year (times subject to change):

Monday September 30th (6th Grade): 3:45-5:30

Tuesday October 1st (7th Grade): 3:45-5:30

Wednesday October 2nd (8th Grade): 3:45-5:30

A first cut will be made after the first days of tryouts are completed, and tryouts for those who make it past the first cut will then resume (times TBA).
Tryouts are closed to everyone but coaches and players. Each day of tryouts is scheduled to end promptly at the announced ending time.....so make sure your student has a ride ready to take him home at that time.
