This form must be read throughly as it contains very important information. Once you and your child have read the document, you must sign and turn the form.  Please be aware that when you sign the form you are agreeing to EVERYTHING in the packet. We also participate in some type of competition throughout the year and there are also team outings that take place. Dates and times to follow.
